• Service Program of the Year (STSP form) (Faith, Family, Community, Life)
• Family of the Year
• Knight of the Year
• Outstanding Young Woman of the Year
• Outstanding Young Man of the Year
• Blessed Michael McGivney Award

Note: The Blessed Michael McGivney Award, two STSP form entries, and two entries from Family of the Year, Knight of the Year, Outstanding Young Woman of the Year, or Outstanding Young Man of the Year meet the End of Year Awards requirement for the State Deputy Award.
