District Deputy and State Staff Information Forms

Following are the forms used to provide information about District Deputies, District Wardens, and State Staff members before the start of a new fraternal year. They can also be updated if information changes throughout the year.

To submit your council’s nominees for the end of year state awards, complete, save, and email the following forms to programsdirector@vakofc.org.

• State Blessed Michael McGivney Award
• Family of the Year
• Knight of the Year
• Young Man of the Year/Young Woman of the Year
