Following are the forms used to provide information about District Deputies, District Wardens, and State Staff members before the start of a new fraternal year. They can also be updated if information changes throughout the year.

• VSS-9 Report Date of Death

Important Notes:

• Clicking the links above opens the state’s secure website Login page. After you log in, you must select the applicable link under the Forms tab.
• Only grand knights and financial secretaries have access to complete the VSS-4, VSS-6, and VSS-9 forms.
• Only district deputies have access to complete the VSS-7 and VSS-8.
• District deputies, grand knights, and financial secretaries can refer to the Virginia State Council Communications Guide for information about how to obtain access to the secure website.

To submit your council’s nominees for the end of year state awards, complete, save, and email the following forms to programsdirector@vakofc.org.
