The Fraternal Programs Report Form (#10784) is a Supreme Council form to submit information about your Council’s service activities. Every program your council does should be reported on this form.

There is no paper version of this form, so online submission is the only option. When accessing the link, the member’s last name and membership number will be required.

The member will receive a link via email to submit the Fraternal Programs Report Form.

After the member submits the form, they will receive a confirmation email and a copy of the form. While the Grand Knight is responsible for all reports created, any council member can submit the report, preferably the member in charge of that program. If the Form 10784 is completed by a council member other than the Grand Knight, it is recommended to forward a copy to the Council’s Grand Knight and the Council’s Program Director.
